APPROVED SOLANO COUNTY VENDOR
FOR REFERRAL PARTNERS
Probation officers and case managers and hospitals and detox partners. Start a referral in under two minutes.
STRUCTURED PLACEMENT Clean and calm shared housing with clear expectations and consistent daily routines from the first night.
COORDINATED HEALTH SUPPORT A structured and safe environment that supports each resident's medication routine and continuity of care. We coordinate directly with each resident's own prescriber and treatment team. We reinforce the plan already in place. We do not replace it.
PARTNER COMMUNICATION We communicate back with referring partners throughout placement. You know how your client is doing at every stage.
BRIDGE TO INDEPENDENCE Residents are supported toward independent living and not just housed and left without a path forward. Every placement has a plan and a direction.
ACCOUNTABILITY STRUCTURE Clear house expectations that support supervision compliance for justice-involved residents. Structured living aligned with probation and parole conditions.
FIT SCREENING We screen every referral against placement criteria before accepting. Placements are appropriate from day one.
SKILLS AND STABILITY TRAINING Residents have access to structured vocational and life-skills training from day one through graduation, running alongside case management. Progress is tracked and documented and not assumed.
OUR COMMITMENT TO REFERRING PARTNERS If a placement isn't working we notify you before we act. We do not remove or transition a resident without first communicating with the referring partner. You are part of the process from referral through exit and not just the handoff.
OUR INTAKE COMMITMENT
24 hours — referral response and bed availability confirmed
48 to 72 hours — placement confirmed and resident received
Day One — case management and program enrollment begins immediately
